Call-outs, overtime and shift overruns
Call-outs, overtime, and shift overruns can disrupt sleep plans, compress recovery, and increase fatigue exposure beyond what a nominal roster suggests. They are common in rail possessions, maintenance, healthcare, construction programmes, and reactive field work.
This page explains fatigue risks from unplanned or extended work. It is not legal advice on overtime pay, working time limits, or whether specific hours are permitted.
Planned overtime vs unplanned overrun

Overtime is not automatically unsafe. The question is whether extended or disrupted patterns remain reasonably practicable to manage with proportionate controls and competent review.
Call-outs after poor sleep
Section titled “Call-outs after poor sleep”Call-outs are particularly demanding because:
- Sleep may be interrupted or foregone entirely
- Workers may attend incidents or sites without full recovery from previous duties
- Alertness may be low during travel and early task phases
- Decision quality under pressure may be affected — human factors research emphasises error risk when fatigued
Organisations with on-call arrangements should assess fatigue risk as part of fatigue risk assessment — not only nominal on-call payment or availability rules.
Early starts after late finishes
Section titled “Early starts after late finishes”Quick returns — finishing late and starting again soon — reduce sleep opportunity. Combined with:
- Early starts before typical wake time
- Night work followed by day commitments
- Travel before the next duty
…they can build cumulative fatigue across a block of work.
Cumulative fatigue and missed breaks
Section titled “Cumulative fatigue and missed breaks”Extended duties often coincide with:
- Missed or shortened breaks during shifts
- Sustained workload without recovery pauses
- Pressure to complete tasks before handover
HSG256 and ORR guidance both emphasise that breaks achievable in practice matter — not only breaks written into a policy.
Workload and operational pressure
Section titled “Workload and operational pressure”Programme pressure, staff shortages, and incident response can normalise overrun. Indicators may include:
- Frequent extensions beyond planned end time
- Overtime clusters before rest days
- Reports of unsustainable pace from supervisors or workers

Return travel after extended work

Recording decisions and worker reporting
Section titled “Recording decisions and worker reporting”When overruns or call-outs elevate fatigue exposure, governance should include:
- Recording what happened and what mitigations were applied — see fatigue decision record template
- Retaining evidence in fatigue records and audit trails
- Encouraging worker fatigue reporting without blame
- Management review when patterns recur — see management accountability for fatigue
Documentation supports learning and review. It does not create automatic approval to work when alertness is impaired.
Post-event review
Section titled “Post-event review”After significant overruns, call-outs, or near-misses, organisations may review:
- Whether roster assumptions remain valid
- Whether staffing, travel, or welfare arrangements need adjustment
- Whether repeated overrun indicates systemic pressure rather than one-off events
